Order 680 Forms

The DH 680 Florida Certification of Immunization is available in the Florida Statewide Health Online Tracking System (FLSHOTS). Though forms on-hand may still be used, only the newer forms are available in bulk. The Florida Department of Health no longer provides the DH 680 computer paper, for use with the Florida State Health Online Tracking System (SHOTS). The DH 680 computer paper is no longer available at the State Distribution Center. Please call 1-877-888-7468 to register with the FLSHOTS immunization registry which will provide access to the electronic version of the DH 680 form.
