Commercial Systems - New
Onsite Sewage Treatment and Disposal Systems are regulated by the Florida Department of Health and the Department of Environmental Protection under Chapter 64E-6 of the Florida Administrative Code.
The following forms are required for a New Permit:
- DH4015-1 Construction Permit Application
- DH4015-3 Soil Evaluation Form
- Site Information Documentation Form
- DH4081 Operating Permit Application
- Survey to Amend Annual Operating Permit
To apply: Download an Application located on the Florida Health Onsite Sewage Forms and Publications web page and submit all required items with permit fee to:

- Application submitted with a Site Evaluation (conducted by certified personnel) - $300.00
- Application submitted without a Site Evaluation (to be conducted by Health Department Staff) - $415.00
Commercial Systems - Repair
Onsite Sewage Treatment and Disposal Systems are regulated by the Florida Department of Health and the Department of Environmental Protection under Chapter 64E-6 of the Florida Administrative Code.

- DH4015-1 Construction Permit Application
- DH4015-4 Existing and Repair Certification
- Site Information Documentation Form
- DH4015-3 Soil Evaluation Form
- Survey to Amend Annual Operating Permit
To apply: Download an Application located on the Florida Health Onsite Sewage Forms and Publications web page and submit all required items with permit fee to:
- Application submitted with a Site Evaluation (conducted by certified personnel) - $250.00
- Application submitted without a Site Evaluation (to be conducted by Health Department Staff) - $365.00
