Existing Systems - Residential
Onsite Sewage Treatment and Disposal Systems are regulated by the Florida Department of Health and the Department of Environmental Protection under Chapter 64E-6 of the Florida Administrative Code.
(If system has been inspected or installed within the last 3 years, we may have the information on file, or; if you have records of repair or installation, you may be able to eliminate this part of the requirement.)
The following forms are required for an Existing System Verification:
- DH4015-1 Construction Permit Application
- DH4015-4 Existing and Repair Certification
- Site Information Documentation Form

New Systems - Residential
Onsite Sewage Treatment and Disposal Systems are regulated by the Florida Department of Health and the Department of Environmental Protection under Chapter 64E-6 of the Florida Administrative Code.

- DH4015-3 Soil Evaluation Form
