How to Obtain a Birth Certificate

  1. In-Person
    • Same-day service is available at:

    • Acceptable IDs: Valid state driver’s license, ID card, passport, or military ID.
  2. By Mail
    • Submit a completed Birth Certificate Application (Spanish) with:
      • A copy of the cardholder’s state-issued ID or driver’s license (if different from the applicant).
      • A self-addressed stamped envelope.

    • Payment: Money orders or cashier’s checks (payable to Vital Statistics). Personal checks are not accepted.
