Please arrive 30 minutes prior to your scheduled appointment. If you are a new patient, please complete a pre-registration appointment for immunizations.
Children must be present for immunization appointments, even when parent/guardian consider children is up-to-date on immunizations.
What to Bring
- Records of previous immunizations. Records from other countries or states are considered valid.
- Non-related adults accompanied 17 years old and younger must present a notarized letter signed by parent/guardian.
- Valid ID for parent/guardian and children. Acceptable IDs:
- Birth certificate (only at first visit)
- Driver’s license
- State issued identification card
- Passport
- Military ID
Services
- Educational materials on immunizations.
- Free immunizations to any child from birth (2 months) through 18 years of age, if eligible.
- Ongoing tracking of infant immunizations to assess completion of the primary immunization series.
- Participation in selected community health fairs dependent upon availability. Educational materials only.
- Proof of Immunization record (DH 680 form needed for school entry/day care), religious exemptions, college forms, and 687 adult clinic record.
