Central Florida Collaborative We want to hear from you! The Central Florida Collaborative is doing a Community Health Needs Assessment to learn about the things going well and things that can be done better to support community health! Your thoughts will help us to learn about health needs, ways to see services, services that may not be easy for you to get, and any issues that you face in seeing health so that we can better meet the needs of you community. Take 15 minutes to complete the survey for a chance to win one of 10 $50 gift cards! *The survey will be open until September 15. Your survey responses & contact information are kept separately. The Central Florida Collaborative has launched the 2025 Community Health Needs Assessment (CHNA) Community Survey for residents of Orange, Osceola, Lake, and Seminole counties.

The survey is available in five languages: English, Spanish, Chinese, Haitian Creole, and Portuguese.

We invite you to participate in the process by helping to identify our community’s current strengths, resources, needs, and service gaps!

Please help us by doing just two things:

  1. Complete the survey using the following the below QR code or link: www.surveymonkey.com/r/cfcsurvey

  2. Download the flyers (PDF 8MB) and the survey link to share with friends and colleagues.

The survey takes only about 15 minutes to complete and the deadline for completion is September 15, 2024.
