If you would like to report a reportable disease or outbreak during business hours, please use the Reportable Disease Form or call the main line in our Epidemiology Office at 407-858-1420.

To report an urgent reportable disease or outbreak after-hours, call 407-858-1420 and follow the prompts to speak to the on-call epidemiologist.
